(1) A record of all vaccinations given under the measles control program should be kept.
(2) Records of successful vaccination and unprotected children will be kept in the office of the concerned area.
(3) The following report regarding vaccination should be submitted as follows:-
(a) In case of engraving done elsewhere than the place of engraving, the person who engraves shall open the details and report to the superintendent concerned,
(b) to submit the vaccination report from each vaccination site to the head office of the wart control program through its area on a monthly basis,
(c) Collect all the grafts received from the grafting place of each area and submit a proper report to the Zonal Office by the concerned supervisor. (d) The Zonal Office will collect the report and send it to the Wafer Elimination Project (e) Sending all the reports collected in the Vifar Eradication Project to the Department of Health Services, Communicable Disease Control Branch.
